How to: Registration Instructions Holliston Youth Cheer

FALL 2022 REGISTRATION OPEN MAY 1ST!

Click on the Green LOG IN button on the upper right hand corner of the website.  Enter your email address and password that you used to register your child.  Then, your child's name will appear and you can click on PRINT FORMS and choose the Official AYF Player Pack.

Listed below are the forms that are required for each member:

1. Participation, Tracking & ID Card (page 1)
2. Participation Contract, Tracking & ID Card (page 2)
3. AYF Code of Conduct Form
4. Waiver & Release of Liability-Minor
5. MTBI/Concussion Annual Statement & Acknowledgement Form
6. Medical Clearance Form or a Physical Form from your doctor dated in 2022
7. Emergency Medical Treatment, Consent & Information
8. Image Release-Minor
9. Report Card - a copy of the participants 2022 year end report card
10. Proof of Age - "NEW CHEERLEADERS ONLY" - Original Birth Certificate, State/City/Town Raised Seal Certified Copy of a Birth Certificate or a Passport must be provided.
